Teen with Painless Jaundice

James is a 16-year-old male (rising 10th grader) on day 4 of antibiotics for a camp-related skin infection from brush scratches; lesions now have minimal redness/tenderness. PMH is notable only for obesity (BMI 32) and he is immunizations up to date. After feeling well yesterday, he developed ~12 hours of new scleral icterus (“yellow eyes”) and orthostatic dizziness with feeling cold and tired, without fever, abdominal pain, N/V/D/C, thirst, or supplement use; appetite and fluids are normal, with recent urination and a formed dark brown stool this morning.
