Coughing, Can't Catch My Breath, Feel Exhausted

A 68-year-old Black male presents with a chief complaint of “coughing nonstop,” “can’t catch my breath,” and feeling exhausted. He reports a five-day history of progressively worsening productive cough with yellow-green sputum, associated with fever, chills, pleuritic chest pain, and dyspnea at rest. Confusion has also been reported. He denies nausea or vomiting but notes poor oral intake and significant fatigue.
